It's because Pedal Power Digital has high current capacity outputs but with Pedal Power 2 Plus, outputs 5 and 6 must be used:

To power the POG with Pedal Power 2 Plus, use output 5 and 6, DIP switches set 9V (normal) and with a Voltage Doubler Cable (reverse polarity, 2.5/2.1mm), part# PPEH24

As is the case with all our power supplies, the earth ground does is not tied to the audio ground. Mostly, the earth ground is merely a safety feature (but may sometimes also have some influence on shielding too).

To power the Boss ES-5 with Pedal Power 2 Plus, use output 5 or 6, DIP switch in the 9V (normal) setting with one of the all black (standard polarity) cables supplied.

Indeed, Pedal Power 3 Plus can power them all (but you'll also need this additional cable HX Current Doubler Cable not included).

Or, similarly with Pedal Power 3 but you'd need to also use an Output Splitter Adapter, part# PPAV to power a pair of 9V pedals from a single output.

Either way, Pedal Power X4 is not required.

To avoid the possibility that an excess of voltage might cause damage, as a general rule, pedals/devices should only be powered with whatever is the proper voltage specified for that pedal/device and not a higher voltage.

That said, for many (but not all) pedals/devices, if the excess is relatively small, it may cause no damage at all (and may even be fine to use).

In the case of the 9V pedals you've got, I would switch to powering them with 9V (and not 12V). As long as they are still working properly, it is extremely unlikely that the modest excess in voltage caused any damage.

Also, most DC pedals/devices require DC power and may be damaged by AC power (or with the wrong polarity DC power).

If it's all working consistently and the Pedal Power 2 Plus is not getting too hot (warm is fine), then it should be okay to continue setup as such.

If
you notice any problems, you might make these adjustments:

To power the Whammy (5) with Pedal Power 2 Plus, use a Current Doubler Adapter, part# PPAP in outputs 4 and 5 (or 6 and 7), DIP switches set normal (9VDC) with one of the all black (standard polarity) cables supplied.

To power the Ground Control Pro with Pedal Power 2 Plus, use a Current Doubler Adapter, part# PPAP in outputs 4 and 5 (or 6 and 7), DIP switches set normal (9VDC) with the supplied red/black ended cable (it's 2.1mm/2.5mm reverse polarity and the red end fits the effect pedal).

Being a voltage "doubler", the cable connects the two 9V outputs in series. As such, the easiest way to check the cable (since you know the outputs are fine) is probably to simply plug the cable into the two 12V outputs and measure the output (red end) of the cable itself.

It should show close to 24VDC. If it shows zero, the cable is likely bad.

It's perfectly fine to power the Pedal Power 3 Plus from the AC courtesy outlet of the Pedal Power 2 Plus.

As for what goes where, here are the details, including, links to additional items which are required (but are not in included with your stock Pedal Power cables):

To power the ProCo Rat (1986 Black Face) use any 9VDC output with 3.5mm Mini Plug cable, part# PPMIN, PPMIN-R

To power the Boss CE-2 (1985 vintage) use any 12V output with one of the all black (standard polarity) cables.

To power the EHX Deluxe Electric Mistress (9V model) use any 9VDC output with one of the all black (standard polarity) cables.
Or,
To power the EHX Deluxe Electric Mistress (24V model) use any two 12V outputs with a Voltage Doubler Cable (reverse polarity, 2.5/2.1mm), part# PPEH24

To power the EHX Stereo Memory Man w/Hazarai use a 9VDC high current capacity (250mA or 500mA) output with one of the all black (standard polarity) cables.

To power the remaining pedals, each:
